We're happy that you have chosen the Universal Academy of Florida as your child(ren)'s school. In order to ensure that your child(ren) are able to attend in August, beginning of the new academic year, please make sure that all of the required paperwork is in order and turned in to the Registrar's office. Document Checklist	Use this sheet to make sure you do not forget to turn in any mandatory documents for the admission of your child(ren).
Registration Form	One form must be filled out by each family. A $50 Registration Fee payment must accompany this form. This payment in non-refundable and goes towards setting up students' files.
Student Information Form	One sheet must be filled out for each child enrolling. Form must be filled out completely and accurately. This must be accompanied by: Immunization Record Physical Exam Report Birth Certificate Social Security Card Legal Guardian Form (For Single Parents)
Records Request Form	One form must be filled out for each child going into grades 1-12 so that we may get records from their previous school.
Authorization of Transportation and Authorization to Walk to Masjid	One of each form must be filled out for each child enrolling.
